Local EMT-1s that require skills testing to re-certify their EMT-1A certification can download the skills sheets listed below. Please study the skills sheets and contact Sierra Ambulance when ready to test. The Evaluator will not mentor or instruct the EMT-1A being tested.

Physician Orders for Life-Sustaining Treatment (POLST)
The POLST Form is a physician order sheet based on the person's current medical condition and wishes. The POLST form should be completed only after discussion with the patient or healthcare decision maker and a physician. The links provided below are for additional information and reference.

Please fill out the Ride-a-long Application bring it in to the business office at 40755 Winding way in Oakhurst during business hours. To schedule a ride-a-long you are required to come in person. Ride-a-longs are required to wear a white button up shirt or polo shirt and black pants and closed toed shoes. Ride-a-longs must be at least 16 years old.
